1. Which of the following most closely describes your current desired field of study? (READ the entire list then choose the best option)
Health Sciences (Dr., Vet, Pharmacy, Polysom, PT, etc.)
Languages (Sign, English etc.)
Math Excellence (must email Lkenyon@collin.edu)
Sciences (other than health)
Technical (HVAC, Welding, Auto, Construction,etc)
Visual Arts (art, game art, photography, etc)
Other (program not listed)
READ ALL before picking one. If you pick wrong, you will be matched with the wrong opportunities and may not get awarded a scholarship. This answer does not have to match your current degree plan through Admissions. Do not put "other" unless none of the other categories fit. FYI: Polysom, Doctor, Pharmacy, Sonography, Veterinarians Kinesiology and Surg Techs are health sciences.
If "other" was chosen to the last question, indicate which field of study?
If you did not select "other" in the previous question, leave this questions blank. Be sure that your major is not listed in the answers to the last question. Putting "other" instead of your desired field of study could eliminate you from being selected for a scholarship. Please describe in one - three words your desired field of study. Do not put AAS, associates, bachelors, doctorate, etc. We want to know what field of study.
If you answered "YES" to the last question, what semester will you be in for the Fall semester,?
For instance, if you started at Collin College 2 years ago but just were accepted and started the Nursing program in Spring, then Fall will be your second semester of the program. This question mainly applies to nurses and other health science programs.
12. Transferring: this Fall will you be transferring from Collin College to one of our partner Universities?
Answering "yes" will eliminate you from receiving a Foundation scholarship for Collin College classes but will place you into another application for our Lebrecht Hites transfer scholarship. You must have a 3.5 GPA or higher to apply. We do not offer scholarships for dual enrolled students.
There are not any opportunities available.
